    摘要:

    目的:探索超声在肝转移性神经内分泌肿瘤(NETLM)鉴别诊断及治疗中的应用。方法:回顾性分析的30例非神经内分泌来源转移性肝癌(NNETLM)患者、29例NETLM患者均在2018年4月至2021年3月期间收治,均进行超声诊断,用ROC曲线分析超声定量指标预测价值,同时对NNETLM患者进行超声引导下消融治疗,分析治疗效果。结果:NETLM组患者的达峰时间(23.52±4.44)s、减退时间(44.55±9.41)s、峰值强度(19.79±3.45)dB高于NNETLM组(P<0.05)。ROC曲线分析,开始增强时间预测NETLM的AUC为0.549,95%区间0.400-0.699,达峰时间预测NETLM的AUC为0.657,95%区间0.517-0.798,减退时间预测NETLM的AUC为0.809,95%区间0.700-0.918,峰值强度预测NETLM的AUC为0.777,95%区间0.656-0.898,四项联合预测NETLM的AUC为0.945,95%区间0.890-1.000。29例患者均顺利完成治疗,26个病灶仅通过1次消融达到肿瘤完全灭活目的。结论:超声能够准确鉴别NETLM,且在超声引导下完成相关治疗,可提高一次性消融成功率,具有较高鉴别价值。

    Abstract:

    Objective: To explore the application of ultrasound in the differential diagnosis and treatment of metastatic neuroendocrine tumors of the liver (NETLM). Methods: A retrospective analysis of 30 patients with non-neuroendocrine metastatic liver cancer (NNETLM) and 29 patients with NETLM were admitted between April 2018 and March 2021, and were diagnosed with ultrasound. ROC curve was used to analyze the predictive value of ultrasound quantitative indicators At the same time, ultrasound-guided ablation treatment was performed on patients with NNETLM to analyze the treatment effect. Results: The peak time (23.52 ± 4.44) s, decline time (44.55 ± 9.41) s and peak intensity (19.79 ± 3.45) dB in netlm group were higher than those in nnetlm group (P < 0.05). According to ROC curve analysis, AUC of netlm predicted by start enhancement time is 0.549, 95% interval is 0.400-0.699, AUC of netlm predicted by peak time is 0.657, 95% interval is 0.517-0.798, AUC of netlm predicted by decline time is 0.809, 95% interval is 0.700-0.918, AUC of netlm predicted by peak intensity is 0.777, 95% interval is 0.656-0.898, AUC of netlm predicted by four items is 0.945, 95% interval is 0.890-1.000. All 29 patients successfully completed the treatment, and 26 lesions were completely inactivated by only one ablation. Conclusion: Ultrasound can accurately identify NETLM, and complete the relevant treatment under the guidance of ultrasound, which can improve the success rate of one-time ablation, and has a high identification value.
