目的 采用左室压力-应变环(left ventricular pressure–strain loop,LVPSL)评估不同左室构型的老年高血压患者心肌做功情况。方法 选取2019年5月至2020年3月在我院心内科门诊就诊的老年高血压患者162例和同时间段35例健康老年人作为对照组进行研究,老年高血压患者根据左室几何构型分为4组,正常构型组(NG)42例,向心性重构组(CR)40例,离心性肥厚组(EH)38例,向心性肥厚组(CH)42例,均接受超声心动图检查并应用EchoPAC软件得出应变值,输入血压值构建左室压力-应变环,获得心肌做功参数。比较各组心肌做功参数。结果 高血压NG组和CR组的GLS与对照组比较,差异无统计学意义(P>0.05),EH组和CH组的GLS小于对照组,差异有统计学意义(P<0.05);高血压各组GWI、GCW、GWW较对照组增大,NG组的GWI、GCW明显大于对照组,差异有统计学意义(P<0.05);与对照组相比,高血压NG组、CR组、EH组的GWE无明显变化,CH组的GWE减小,差异有统计学意义(P<0.05)。结论 LVPSL可定量评价不同左室构型受试者的心肌做功情况,评估高血压患者左心室收缩功能更为准确、全面,为临床早期诊断及预后判断提供依据。 |

Objective Assessment of myocardial work in elderly hypertensive patients with different left ventricular configuration using Left ventricular pressure-strain loop. Methods From May 2019 to March 2020, 162 elderly hypertensive patients and 35 healthy elderly patients were selected as the control group for the study. Hypertensive patients were divided into 4 groups according to left ventricular geometry configuration, normal configuration group (42 cases), centripetal remodeling group (40 cases), centrifugal hypertrophy group (38 cases) and centrifugal hypertrophy group (42 cases). All echocardiograms were performed and Echo PAC software to obtain strain, and inputting pressure to construct LV pressure-strain loops to obtain myocardial work parameters. Myocardial work parameters were compared . Results Compared with the control group, the GLS of the hypertension NG group and the CR group had no significant difference (P>0.05), but the GLS of the EH group and the CH group was lower than that of the control group, the difference was statistically significant (P<0.05). The GWI, GCW, and GWW of each hypertension group were higher than those of the control group, and the GWI and GCW of the NG group were significantly greater than those of the control group, the difference was statistically significant (P<0.05). Compared with the control group, the GWE of the hypertensive NG group, CR group, and EH group did not change significantly, while the GWE of the CH group decreased, the difference was statistically significant(P<0.05). Conclusion LVPSL can evaluate the myocardial work of subjects with different left ventricular configurations, and evaluate left ventricular systolic function in hypertensive patients more accurately and comprehensively, providing a basis for early clinical diagnosis and prognosis judgment. |
