【摘要】目的:探讨伴鼠类肉瘤病毒癌基因同源物B1(v-raf murine sarcoma viral oncogene homologue B1,BRAF)突变甲状腺乳头状癌(Papillary carcinoma of thyroid,PTC)患者超声特征及与淋巴结转移的关系。方法:本次研究纳入绵阳市第三人民医院2015年1月-2022年1月收治PTC患者共417例,均经穿刺或手术病理组织学检查确诊,分析不同亚型患者BRAF V600E突变阳性率和淋巴结转移情况关系,采用Logistic回归模型评价BRAF V600E突变对不同亚型患者超声图像特征的影响。结果:①高细胞亚型BRAF V600E突变阳性率显著高于经典亚型、滤泡亚型(P<0.05);②BRAF V600E突变阳性人群不同亚型颈部淋巴结转移率比较差异有统计学意义(P<0.05);不同亚型BRAF V600E突变阳性患者颈部淋巴结转移率显著高于BRAF阴性者(P<0.05);③合并BRAF V600E突变不同亚型患者钙化类型和是否贴近被膜比例比较差异有统计学意义(P<0.05);④多因素分析结果显示,BRAF V600E突变与经典亚型/高细胞亚型患者钙化特征独立相关(P<0.05)。结论:合并BRAF V600E突变PTC患者出现颈部淋巴结转移几率更高;BRAF V600E突变存在可影响经典亚型/高细胞亚型PTC患者超声图像钙化类型。 |

Abstract:Objective To investigate the ultrasonographic characteristics of PTC patients with BRAF V600E mutation and its relationship with lymph node metastasis. Methods Clinical data of 417 patients with PTC confirmed by puncture or surgery histopathology were retrospectively chosen in the period from January 2015 to January 2022. The relationship between the positive rate of BRAF V600E mutation and lymph node metastasis of patients with different subtypes was analyzed and the influence of BRAF V600E mutation on the ultrasonic image characteristics of patients with different subtypes was evaluated by Logistic regression model. Results ①The BRAF V600E mutation positive rate of TCV group was significantly higher than CV group and FV group(P<0.05). ②There were significant difference in cervical lymph node metastasis rate among different subtypes of BRAF V600E mutation positive population(P<0.05). The cervical lymph node metastasis rate of BRAF V600E mutation positive patients with different kinds of subtypes were significantly higher than BRAF V600E mutation negative(P<0.05). ③There were significant difference in the proportion of calcification types and calcification close to the capsule between different kinds of subtypes(P<0.05). ④Multivariate analysis showed that BRAF V600E mutations were independently associated with calcification of patients with CV and TCV subtypes(P<0.05). Conclusion PTC patients with BRAF V600E mutation possess the higher incidence of cervical lymph node metastasis and the presence of BRAF V600E mutation can affect the calcification type of ultrasound image in PTC patients with CV/ TCV subtype. |
