目的: 应用二维斑点追踪成像技术(2D-STI)评价急性前壁心肌梗死患者经皮冠状动脉介入治疗(PCI)前后右心室功能的临床价值。方法:选取我院接受支架植入术的急性前壁心肌梗死患者36例为病例组,选取40名冠脉造影阴性的检查者为对照组,观察PCI术前、术后1月和对照组右心室二维应变参数。结果:PCI术前、术后1月、对照组的左室射血分数、右室(心内膜和心肌)收缩期整体纵向峰值应变、右室游离壁(基底段和中间段)收缩期径向峰值应变、室间隔(心尖段和中间段)收缩期径向峰值应变、右室游离壁(中间段和心尖段)收缩期纵向峰值应变、室间隔(心尖段)收缩期纵向峰值应变、右室游离壁中间段收缩期径向峰值应变率、室间隔心尖段收缩期径向应变率的绝对值逐渐提高(均P<0.05);PCI术后1月的右室应变参数较术前有不同程度的提高。结论:二维斑点追踪成像技术可定量评估急性前壁心肌梗死患者PCI术前后右心室功能,PCI术后患者的右心室功能较术前有明显的改善。 |

Objective: To assess right ventricular (RV) function of patients with acute anterior wall myocardial infarction before and after percutaneous coronary intervention (PCI) by two-dimensional speckle tracking imaging(2D-STI). Methods: Totally 36 patients with acute anterior wall myocardial infarction underwent successful PCI were included in case group,40 patients without coronary angiography were included in the control group, the two-dimensional strain parameters of right ventricle before PCI、1month after PCI and the control group were observed. Results: The absolute of left ventricular ejection、the right ventricular global longitudinal strain 、 the right ventricular(free-wall basal and med)radial strain、septum(apical and med)radial strain、the free-wall med and apical longitudinal strain of right ventricular、septum apical longitudinal strain、the free-wall med radial strain rate of right ventricular and septum apical radial strain rate increased gradually(all P<0.05). the parameters showed a trend of improvement compared with those one month after PCI. Conclusion: 2D-STI can quantitatively evaluate right ventricular function of patients with acute anterior wall myocardial infarction before and after PCI, there was a significant improvement of right ventricular function after PCI. |
