目的:分析三维超声自由解剖成像技术(OmniView)及断层超声成像技术(tomographic ultrasound imaging,TUI)对宫腔粘连(intrauterineadhesions,IUA)的评估价值。方法:选取本院2021年3月~2022年3月期间收治的112例疑似IUA患者,均行OmniView及TUI扫描检查。患者均在本院接受治疗,将宫腔镜结果作为金标准,分析OmniView及TUI对IUA及其分型的检测效能,采用受试者工作特征曲线(ROC)评估OmniView及TUI对IUA的评估效能。结果:宫腔镜检出IUA 98例,非IUA 14例。粘连分型中,中央型62例,周围型11例,混合型25例。OmniView检出IUA患者93例,非IUA 患者19例;中央型59例,周围型10例,混合型24例。IUA检出IUA患者88例,非IUA患者24例;中央型58例,周围型7例,混合型23例。根据Kappa一致性检验,OmniView及TUI与宫腔镜检测结果一致性一般(Kappa=0.681、0.625),联合检测与病理检测结果一致性较好(Kappa=0.813)。根据ROC模型,OmniView及TUI对IUA均具有一定诊断效能,联合检测效能最高,其诊断AUC、灵敏度、特异度和准确性分别为0.939、95.92%、92.86%、95.54%。结论:OmniView及TUI均可用于检测IUA及其分型,其中二者联合检测的效能最高,联合检测结果与宫腔镜结果一致性较好,可用于临床筛查IUA。 |

Objective: To analyze the value of three-dimensional ultrasound free anatomy imaging (OmniView) and tomographic ultrasound imaging (TUI) in evaluating intrauterine adhesions (IUA). Methods: A total of 112 patients with suspected IUA admitted to our hospital from March 2021 to March 2022 were selected, and all underwent OmniView and TUI scans. All patients were treated in our hospital. Taking the results of hysteroscopy as the gold standard, the detection performance of OmniView and TUI for IUA and its classification was analyzed. The receiver operating characteristic curve (ROC) was used to evaluate the performance of OmniView and TUI for IUA. Results: Hysteroscopy detected IUA in 98 cases and non-IUA in 14 cases. Among the adhesion types, 62 cases were central type, 11 cases were peripheral type, and 25 cases were mixed type. OmniView detected 93 IUA patients and 19 non-IUA patients; 59 central type, 10 peripheral type, and 24 mixed type. IUA detected 88 IUA patients and 24 non-IUA patients; 58 central type, 7 peripheral type, and 23 mixed type. According to the Kappa consistency test, the results of OmniView, TUI and hysteroscopy were generally consistent (Kappa=0.681, 0.625), and the results of combined detection and pathology were better (Kappa=0.813). According to the ROC model, both OmniView and TUI have certain diagnostic efficacy for IUA, and the combined detection efficacy is the highest. Conclusion: Both OmniView and TUI can be used to detect IUA and its classification. The combined detection of the two has the highest efficiency, and the combined detection results are in good agreement with the results of hysteroscopy, which can be used for clinical screening of IUA. |
