目的 探讨超声测量视神经鞘直径(ONSD)联合闪光视觉诱发电位(FVEP)在颅脑损伤患者颅内压评估中的应用价值。方法 回顾性分析我院2020年1月至2022年6月收治的100例颅脑损伤患者一般资料,均给予FVEP检测颅内压,并根据患者是否联合ONSD分组,将仅采用FVEP检测者纳入对照组(n=43),将联合FVEP、ONSD检测者纳入联合组(n=57),对比两组脱水剂使用情况、急性肾损伤(AKI)发生率,记录两组出院及恢复情况。结果 100例颅脑损伤患者中有51例出现颅内压升高,另外49例患者颅内压正常,两组颅内压升高者颅内压值比较无显著差异(P>0.05),联合组中颅内压升高者ONSD值显著高于颅内压正常者(P<0.05);对照组中颅内压升高者颅内压显著高于正常者(P<0.05),但两组颅内压值比较无显著差异(P>0.05);经相关性分析发现,ONSD值、颅内压值与颅内压升高均呈正相关,且二者之间亦呈正相关(P<0.05);联合组甘露醇使用时间及总量均显著低于对照组,急性肾损伤例数亦低于对照组(P<0.05);联合组住院费用、抗菌药物使用强度均较对照组更低,住院时间、机械通气时间均明显短于对照组(P<0.05);联合组治愈率为66.67%显著高于对照组44.19%(P<0.05)。结论 ONSD联合FVEP在颅脑损伤患者颅内压监测中具有重要意义,还可指导临床用药,促进恢复,值得推广。 |

Objective To investigate the application value of ultrasoundmeasuredoptic nerve sheath diameter (ONSD) combined with flash visual evoked potential (FVEP) in evaluating intracranial pressure of patients with craniocerebral injury. Methods The general data of 100 patients with craniocerebral injury admitted to the hospital from January 2020 to June 2022 were retrospectively analyzed. FVEP was used to detect the subjects’intracranial pressure. According to the use of ONSD, the subjects were divided into control group (n=43, without use of ONSD) and combination group (n=57, with ONSD). The use of dehydrating agents and the incidence of acute kidney injury (AKI) were compared between the groups.Discharge and recovery were recorded. ResultsIn this study, there were 51 patients with increased intracranial pressure and 49 patients with normal intracranial pressure.Patients with increased intracranial pressure in the two groups had similar intracranial pressure (P>0.05). In the combination group, patients with increased intracranial pressure had larger ONSD than those with normal intracranial pressure (P<0.05). The two groups had similar intracranial pressure (P>0.05). Correlation analysis showed that ONSD was positively correlatedintracranial pressure(P<0.05). The usage time and total dosage of mannitol in the combination group were significantly shorter/lower than those in the control group. Patients with AKI were fewer than the control group (P<0.05). The hospitalization cost and use intensity of antibiotics inthe combination group were lower than those in the control group. The length of hospital stay and mechanical ventilation time were significantly shorter than those of the control group (P<0.05). The cure rate inthe combination group was 66.67%, significantly higher than 44.19% inthe control group (P<0.05). ConclusionONSD combined with FVEP is of great significance for monitoring intracranial pressure of patients with craniocerebral injury, which also can guide clinical medication and promote recovery. |
