目的探究经会阴剪切波弹性成像联合前腔室结构参数在自然分娩产妇盆腔器官脱垂(POP)筛查中的应用。方法选取2020年5月至2022年10月在我院自然分娩且产后42 d接受复查的初产妇91例,其中44例发生POP为观察组,47例未发生POP为对照组,均行经会阴盆底常规超声及剪切波弹性成像,获取产妇静息状态和最大瓦尔萨尔瓦(Valsalva)状态时前腔室结构参数和肛提肌弹性模量。结果静息状态下,观察组UTA、BNS小于对照组,PUA大于对照组(均P<0.05);Valsalva状态下,观察组UTA、PUA大于对照组,BNS小于对照组(均P<0.05);观察组RUA大于对照组,差异有统计学意义(P<0.05)。观察组肛提肌附于耻骨下支处、肌腹及后下方△Emax和△Emean小于对照组,差异均有统计学意义(P<0.05)。剪切波弹性成像联合前腔室结构参数诊断的灵敏度为88.64%(39/44),特异度为95.74%(45/47),准确度为92.31%(84/91)。结论自然分娩产妇POP后肛提肌收缩功能降低,经会阴剪切波弹性成像测量肛提肌、联合前腔室结构参数有助于产后POP的筛查。 |

Abstract: |
Objective To explore the application of transperineal shear wave elastography combined with anterior chamber structure parameters in the screening of pelvic organ prolapse (POP) for puerperae undergoing natural delivery. Methods A total of 91primiparas undergoing reexamination at 42d after natural delivery in the hospital were enrolled between May 2020 and October 2022, including 44cases with POP in observation group and 47cases without POP in control group. Allunderwent routine ultrasound and shear wave elastography to obtain anterior chamber structure parameters and elastic modulus of levator ani in rest state and maximum Valsalva (Valsalva) state. Results In rest state, UTA and BNS in observation group were lower than those in control group, while PUA was greater than that in control group (P<0.05). In Valsalva state, UTA and PUA in observation group were greater than those in control group, while BNS was lower than that in control group (P<0.05). RUA in observation group was greater than that in control group(P<0.05), maximum elastic modulus (△Emax) and mean elastic modulus (△Emean) attached to the inferior ramus of pubis, muscular abdomen and the posterior site of levator ani were lower than those in control group(P<0.05). The diagnostic sensitivity, specificity and accuracy of shear wave elastography combined with anterior chamber structure parameters were 88.64% (39/44), 95.74% (45/47) and 92.31% (84/91),respectively. Conclusion The systole function of levator ani is declined in natural delivery puerpera after POP. The measurement of levator ani by shear wave elastography combined with anterior chamberstructure parameters is conducive to the screening of postpartum POP. |
