目的 分析非小细胞肺癌(non-small cell lung cancer, NSCLC)颈部转移性淋巴结的超声造影(contrast-enhanced ultrasound, CEUS)定性特征和定量参数与Ki-67表达水平的相关性。方法 选取2021年6月至2022年7月期间三峡大学第一临床医学院宜昌市中心人民医院的125例NSCLC患者进行回顾性研究,最终纳入52例患者的经穿刺活检病理证实的60枚NSCLC颈部转移性淋巴结,根据淋巴结Ki-67表达量分为高表达组(Ki-67≥30%)42枚和低表达组(Ki-67<30%)18枚,比较两组淋巴结的常规超声特征、CEUS定性特征(灌注模式和灌注类型)以及中央区和周边区的CEUS定量参数,分析CEUS定量参数预测Ki-67表达水平的能力。结果 Ki-67高表达组淋巴结整体及中央区的峰值强度PI、灌注速度C和周边区峰值强度PI均大于低表达组,差异有统计学意义(均P<0.05)。Ki-67高表达组淋巴结中央区的血流量(AUC)高于低表达组,差异有统计学意义(P<0.05),两组周边区的AUC无显著差异。淋巴结中央区的AUC和C与Ki-67表达量均呈正相关(均P<0.001),中央区AUC和C预测Ki-67表达水平的ROC曲线下面积分别为0.8440和0.9001。两组淋巴结的常规超声特征和CEUS定性特征均无统计学差异(均P>0.05)。结论 NSCLC颈部转移性淋巴结的CEUS定量参数中央区AUC和C可以预测Ki-67的表达水平,有助于临床无创评价NSCLC的肿瘤生物学特性。 |

Abstract: |
Objective: To analyze the correlation between the qualitative characteristics and quantitative analysis of contrast enhanced ultrasound (CEUS) and the expression of Ki-67 in cervical metastatic lymph nodes of non-small cell lung cancer (NSCLC).SMethods From June 2021 to July 2022, 125 patients with NSCLC in Yichang Central People"s Hospital were selected for retrospective study. Finally, 60 cervical metastatic lymph nodes of 52 patients with NSCLC confirmed by biopsy and pathology were included. According to the expression of Ki-67 in lymph nodes, they were divided into high expression group (Ki-67≥30%) and low expression group (Ki-67<30%). The ultrasound characteristics of lymph nodes in the two groups were compared,including CEUS qualitative characteristics (perfusion mode and perfusion type) and CEUS quantitative parameters of medulla Central and surrounding areas.They were analyzed to discuss the ability of CEUS quantitative parameters to predict Ki-67 expression level.Results The peak intensity (PI), perfusion velocity (C) of CEUS quantitative parameters in the whole lymph node and central area with the PI of peripheral area in Ki-67 high expression group were higher than those in low expression group (all P<0.05). The blood flow (AUC) of central area of lymph nodes in Ki-67 high expression group was higher than that in low expression group (P<0.05), and there was no significant difference in AUC of periperal area between the two groups. AUC and C in central region of lymph nodes were positively correlated with Ki-67 expression (all P<0.001). The area under ROC curve of central region AUC and C predicting Ki-67 expression was 0.8440 and 0.9001, respectively. There was no significant difference between the two groups in the conventional ultrasound characteristics and CEUS qualitative characteristics of lymph nodes (all P>0.05). Conclusion The CEUS quantitative parameters AUC and C in central region of cervical metastatic lymph nodes of NSCLC can predict the expression level of Ki-67, which is helpful to evaluate the tumor biological characteristics of NSCLC in a non-invasive manner. |
