目的 探究二维剪切波弹性成像技术(2D-SWE)评估高血压肾病小鼠肾纤维化的应用价值。方法 将C57小鼠随机分为正常对照组、模型组。正常对照组10只使用微渗泵灌注生理盐水,模型组使用用微渗透泵灌注血管紧张素Ⅱ,模型组根据建模时间长短,分为高血压肾病Ⅰ组、Ⅱ组、Ⅲ组、Ⅳ组,每组各5只。测量造模前后小鼠血压变化情况;通过2D-SWE技术测定不同造模阶段小鼠的肾皮质硬度;透射电镜观察肾小球毛细血管基底膜结构;酶联免疫法检测尿素氮(urea nitrogen,BUN)、血肌酐(serum creatinine,Scr)和24 h尿蛋白定量(24 h urinary protein,24 h UP);HE染色和Masson染色检测肾组织病理损伤;Western Blot检测肾组织内α-平滑肌肌动蛋白(α-smooth muscle actin,α-SMA)、I型胶原蛋白(collagen-Ⅰ)、Ⅲ型胶原蛋白(collagen-Ⅲ)表达。结果 造模结束后小鼠血压明显上升;透射电镜观察到肾组织毛细血管内皮细胞损伤明显,基底膜节段性皱缩;其中高血压肾病Ⅲ组、Ⅳ组2D-SWE技术检测显示小鼠的肾皮质硬度测值显著升高;HE和Masson染色显示小鼠肾组织内肾小管损伤严重,甚至空泡变性,上皮细胞脱落,炎性细胞大量浸润,并伴随明显的纤维化;Western Blot结果显示 α-SMA、collagen-Ⅰ、collagen-Ⅲ的蛋白表达水平均升高。结论 2D-SWE技术检测的小鼠肾皮质硬度与小鼠肾纤维化程度呈正相关,2D-SWE技术对高血压肾病肾纤维化的评估具有重要的临床应用价值。 |

Application of two-dimensional shear wave elastography to assess renal fibrosis in mice with hypertensive nephropathy |

Abstract: |
Objective To investigate the application value of two-dimensional shear wave elastography (2D-SWE) in assessing renal fibrosis in mice with hypertensive nephropathy. Methods C57 mice were randomly divided into normal control and model groups. Ten normal control mice were perfused with saline using a microosmotic pump, and the model group was perfused with angiotensin II using a microosmotic pump. The model group was divided into hypertensive nephropathy group I, II, III and IV according to the duration of modeling, with 5 mice in each group. The changes of blood pressure were measured before and after modeling; the renal cortical stiffness of mice at different modeling stages were measured by 2D-SWE technique; the glomerular capillary basement membrane structure was observed by transmission electron microscopy; urea nitrogen (BUN), serum creatinine (Scr) and 24 h urinary protein (24 h UP) were quantified by enzyme-linked immunoassay. The expression of α-smooth muscle actin (α-SMA), collagen type I (collagen-Ⅰ) and collagen type III (collagen-Ⅲ) in renal tissues was detected by Western Blot. collagen-III) expression in kidney tissues. Results The blood pressure of mice increased significantly after the end of modeling; the damage of capillary endothelial cells and segmental wrinkling of basement membrane in kidney tissues were observed by transmission electron microscopy; the 2D-SWE technique in groups III and IV of hypertensive nephropathy showed a significant increase of renal cortical stiffness measurement in mice; HE and Masson staining showed severe tubular damage, even vacuolar degeneration, epithelial cell detachment and inflammatory Western Blot showed that the protein expression levels of α-SMA, collagen-Ⅰ and collagen-Ⅲ were increased. Conclusion The renal cortical stiffness detected by 2D-SWE technique was positively correlated with the degree of renal fibrosis in mice, and 2D-SWE technique is of great clinical application for the assessment of renal fibrosis in hypertensive nephropathy. |
