【】: 目的:探讨胎儿心脏定量分析技术(fetal HQ)在评估妊娠期高血压疾病(Hypertensive disorder complicating pregnancy, HDCP)胎儿心脏形态与功能中的应用。方法 :选择2022年3月到2022年11月于山西省妇幼保健院孕28-40W行正规产前超声检查并分娩的49例HDCP孕妇为病例组,同期52例正常孕妇为对照组。对两组病人进行前瞻性观察研究,应用Fetal HQ软件测量胎儿四腔心切面舒张末期整体球形指数(Global sphericity index,GSI),通过斑点追踪技术分析计算心室面积变化分数(fractional area change,FAC),同时获取左、右心室24节段短轴缩短率(FS)的Z-评分及24节段SI。采用Pearson相关分析对照组孕周与GSI、FAC、SI间的相关性。计量资料两组间采用t检验,不符合正态分布的计量资料采用Wilcoxon符号秩检验,对比分析两组间各数值间的差异。结果:对照组孕周与GSI、左右心室FAC及左右心室24SI间的相关性均无统计学意义(P>0.05)。HDCP组GSI指数与对照组比较差异具有统计学意义(P<0.05)。HDCP组左右心室FAC指数与对照组比较差异均具有统计学意义(P<0.05)。HDCP组与对照组左、右心室FS的Z评分相互比较,左心室2-24节段有明显统计学意义(P<0.05),右心室1-24节段有明显统计学意义(P<0.05),HDCP组左右心室FS的Z评分均明显小于对照组胎儿。HDCP组与对照组左、右心室24节段SI值相互比较,左心室2-16节段SI有明显统计学意义(P<0.05),左心室1、17-24节段SI无明显统计学意义,右心室1-24节段SI无明显统计学意义(P>0.05)。结论:Fetal HQ可用于评估HDCP胎儿心脏形态与功能。 |

【】? Objective: To investigate the application of fetal HQ technique in the assessment of fetal heart morphology and function in hypertensive disorder complicating pregnancy (HDCP). Methods : Forty-eight pregnant women with HDCP who underwent regular antenatal ultrasound examination and delivered at the Shanxi Maternal and Child Health Hospital 28-40W from March 2022 to November 2022 were selected as the case group, and 52 normal pregnant women were selected as the control group during the same period. The fractional area change (FAC) was calculated by speckle tracking technique, and the left and right ventricular shortening rates (GSI) were also obtained. The correlation between gestational week and GSI, FAC and SI in the control group was analysed by Pearson correlation. The t-test was used for measurement data between the two groups, and the Wilcoxon signed-rank test was used for measures that did not conform to a normal distribution, to compare and analyse the differences between the values between the two groups. Results: The correlation between gestational week and GSI, right and left ventricular FAC and right and left ventricular 24SI in the control group was not statistically significant (P>0.05). the difference between the GSI index in the HDCP group and the control group was statistically significant (P<0.05). the difference between the right and left ventricular FAC index in the HDCP group and the control group was statistically significant (P<0.05 The Z scores of left and right ventricular FS in the HDCP group were statistically significant (P<0.05) when comparing the left and right ventricular FS with each other, and the right ventricular FS was statistically significant (P<0.05) when comparing the right and left ventricular FS in the HDCP group with the control group.The Z scores of left and right ventricular FS in the HDCP group were statistically significant (P<0.05) when comparing the left and right ventricular FS with each other, and the right ventricular FS was statistically significant (P<0.05) when comparing the left and right ventricular FS in the HDCP group with the control group. SI values were statistically significant (P<0.05) when comparing each other, SI of left ventricular segments 2-16, SI of left ventricular segments 1 and 17-24 were not statistically significant, and SI of right ventricular segments 1-24 were not statistically significant (P>0.05). CONCLUSION: Fetal HQ can be used to assess the morphology and function of the HDCP fetal heart. |
