Abstract:Objective To investigate the clinical value of the volume of the whole stomach(VWS) measured by conventional ultrasound in evaluating gastric accommodation function in patients with diabetes mellitus,and to analyze its related influencing factors.Methods 121 diabetic patients (diabetes group) and 99 healthy volunteers (healthy control group) in our hospital were selected.After drinking 500 ml warm water,the filled gastric cavity was examined by ultrasound scan.The maximum section area of gastric fundus,the short axis section area and long diameter of gastric body,and the short axis section area and long diameter of gastric antrum were measured,and VWS was calculated.According to the gender,age,BMI,fasting blood glucose (FPG) and hemoglobin a1c (HbA1c) levels of the enrolled diabetic patients,the differences of VWS related ultrasound values between the diabetic group and the healthy control group and the diabetic patients with different clinical data were compared.The correlation between VWS and gender, age, BMI, FPG and HbA1c was analyzed.Results The VWS of diabetic group was (516.03±73.42) cm3,and that of healthy control group was (679.23±96.25) cm3.Gastric fundus area (FA),gastric body length diameter (BL),gastric body area (BA),gastric antrum area (AA),and VWS in diabetic group were all lower than those in healthy control group,and the differences were statistically significant (all P < 0.05).Comparison of diabetic patients with different clinical data: ① Compared with male group,FA and VWS in female group decreased,and the differences were statistically significant (all P < 0.05); There was no significant difference in BL,BA,AL,and AA.② There were statistically significant differences in FA,BA,and VWS among diabetic patients with different FPG levels (all P < 0.05).Compared with FPG < 7 mmol/L group and 7 mmol/L≤FPG < 10 mmol/L group,FA, BA,and VWS in FPG≥10 mmol/L group were decreased (all P < 0.05).Compared with FPG < 7 mmol/L group,FA and VWS in 7 mmol/L≤FPG < 10 mmol/L group were decreased (all P < 0.05).③ There was no significant difference in the ultrasonic measurements of diabetes patients with different age,BMI,and HbA1c levels. Correlation analysis showed that VWS was negatively correlated with FPG (rs=-0.411, P < 0.01),but had no correlation with gender,age,BMI,and HbA1c.Conclusions Conventional ultrasound measurement of VWS is expected to be an effective means to evaluate gastric accommodation in diabetic patients.The VWS of diabetic patients was lower than that of healthy volunteers,and FPG was the main influencing factor of VWS in diabetic patients.
