目的 探究同种异体肾移植后肾皮质微灌注变化与超声造影(Contrast-enhanced Ultrasound,CEUS)定量参数特征,为肾移植的CEUS评估提供循证依据。方法 将主题词与相关自由词组合,在Embase、MEDLINE、Web of Science、Cochrane Library、中国知网、万方、维普等数据库进行文献检索(2000-2023)。收集有关CEUS评估同种异体肾移植术后肾皮质微循环的相关研究。按照PRISMA指南进行荟萃分析和系统综述。结果 共纳入11篇文献(977例肾移植患者)。纳入研究的总体质量为中到高等。荟萃分析显示,肾功能障碍组超声造影参数曲线下面积 (SMD: -1.01, 95% CI: -1.38 ~ -0.63,P<0.05) 及峰值强度(SMD:-0.55, 95% CI:-1.02 ~ -0.08, P = 0.02)明显低于肾功能稳定组。达峰时间(Time to peak intensity, TTP)较肾功能稳定组明显延长(SMD:0.33, 95% CI:0.02 ~ 0.63, P = 0.04)。结论 同种异体肾移植术后肾功能障碍的患者皮质微循环灌注总量减少,肾皮质灌注强度降低,灌注时间延长,CEUS参数可以及时的反应肾皮质微灌注的变化,为移植术后肾功能障碍的早期诊断提供依据。 |

Objective Exploring the changes in renal cortical microcirculation and quantitative parameter characteristics of contrast-enhanced ultrasound (CEUS) after allogeneic kidney transplantation, providing evidence-based evaluation for CEUS in kidney transplantation. Methods Combine theme words with relevant free words and conduct literature searches on databases such as Embase, MEDLINE, Web of Science, Cochrane Library, CNKI, Wanfang, and VIP (2000-2023). Collect relevant studies on contrast-enhanced ultrasound evaluation of renal cortical microcirculation after allogeneic kidney transplantation. Perform a meta-analysis and systematic review according to the PRISMA (Preferred Reporting Items for systematic reviews and meta-analysis) guidelines. Results 11 studies(977 kidney transplant patients) were included. The overall quality included in the study ranges from medium to high. Meta analysis showed that the area under the curve (SMD: -1.01, 95% CI: -1.38~-0.63) and peak intensity (SMD: -0.55, 95% CI: -1.02~-0.08) of ultrasound contrast parameters in the renal dysfunction group were significantly lower than those in the stable renal function group. The peak intensity (PI) of the group with renal dysfunction was significantly lower than that of the group with stable renal function (SMD: -0.55, 95% CI: -1.02~ -0.08). The time to peak intensity (TTP) was significantly longer in the group with stable renal function (SMD: 0.33, 95% CI: 0.02~0.63). Conclusion Patients with renal dysfunction after allogeneic kidney transplantation have reduced total cortical microcirculation perfusion, decreased renal cortical perfusion intensity, and prolonged perfusion time. CEUS parameters can timely reflect changes in renal cortical microcirculation, providing a basis for early diagnosis of renal dysfunction after transplantation. |
