| 目的 探讨卵巢子宫内膜样癌(ovarian endometrioid carcinoma,OEC)的临床特征及超声征象,以提高对OEC的认识及超声诊断能力。 方法 回顾性分析经我院病理确诊的18例OEC患者的临床及超声资料,总结OEC的临床特征及超声征象。 结果 18例OEC患者中,17例单灶,1例双灶,其中8例同步子宫内膜样癌,年龄24-59岁,中位年龄48岁,其中未绝经13例,未育5例；CA125、CA19-9、HE4三者同时升高者10例；盆腔或淋巴结转移4例。19个病灶中,最大直径34-233mm,中位最大直径113mm,囊实性病灶18个、实性1个,边界清楚13个、不清楚6个,形态规则5个、不规则14个,血供较丰富15个、少量血供4个；另外,18个囊实性病灶中,囊性部分的囊壁表现为增厚且厚薄不均9个、囊壁毛糙8个、囊壁尚光滑1个,囊内有低弱细点状回声16个、囊内透声尚好2个,囊内有带状分隔且厚薄不均9个、隔带厚度较均匀7个、无隔带2个,囊内实性部分均呈不规则状,另外囊内有乳头状突起4个；此外,12例患者出现腹盆腔积液,2例腹水内有癌细胞。? 结论 OEC患者临床及超声表现有一定特征,两者结合有助于早期诊断OEC。 |

| 【】 Objective To investigate the clinical features and ultrasound manifestations of aovarian endometrioid carcinoma(OEC) in order to improve the recognize and the ability of ultrasound diagnosis of OEC. Methods The clinical and ultrasonic data of 18 patients with OEC confirmed by pathology in our hospital were retrospectively analyzed, and the clinical features and ultrasound characteristics of OEC were summarized. Results? Among 18 patients of OEC,17 were unilateral and 1 was bilateral,and 8 of these patients also had a synchronous endometrial cancer,the median age was 48 (range 24-59) years and 5 patients were nulliparous and 13 patients were premenopausal;there were 10 of serum CA125、CA19-9 and HE4 were elevated simultaneously；pelvic or lymph node metastasis in 4 cases.The masses median largest diameter was 113 (range 34-233)mm,18 of them were mixed cystic-solid and the other one was solid,13 masses with clear boundary and 6 others appeared boundary ambiguity,the shape regular and irregular has 5 cases and 14 cases respectively, the blood flow was abundant and a little has 15 cases and 4 cases respectively;in addition,among 18 masses of mixed cystic-solid,there were 9 cases with thickening and uneven thickness,8 cases with rough wall and 1 case with smooth wall,16 cases cystic with low-weak echo and the 2 others has clear echo,inside the cyst has 9 bands with uneven thickness, 7 with even thickness and 2 without septa,all of the solid part of the mixed cystic-solid masses |
