    目的:探讨基于超声造影特征构建列线图模型对肝内胆管细胞癌(ICC)与LR-M类肝细胞癌(HCC)的鉴别诊断价值,并评估列线图模型的诊断效能。方法:回顾性分析经病理证实的 ICC 和 HCC 患者的超声造影(CEUS)数据 ,所有结节进行CEUS LI-RADS分类,以结节大小进行倾向评分匹配(ICC:LR-M类HCC=1:1),并按7:3分为训练集和验证集。应用R软件,采用Logistic分析筛选ICC最具特征的CEUS征象,构建列线图模型,绘制ROC曲线、校准曲线及决策曲线对该模型进行评价。结果:与LR-M类的HCC相比,ICC最具特征性CEUS征象为:动脉期周边环状增强、瘤内静脉、瘤内未增强区边界模糊、肿瘤增强边界不清,构建CEUS特征的列线图模型,其ROC曲线的AUC 分别为0.9062(训练集)、0.8908(验证集),模型区分度良好;校准曲线显示列线图模型预测概率与实际病理结果一致性良好。临床决策曲线显示模型具有临床意义。结论:基于超声造影特征构建的列线图模型对ICC与LR-M类HCC的鉴别诊断具有较高的临床价值。

    Abstract:

    Objective: To evaluate the diagnostic value of nomogram based on contrast-enhanced ultrasound (CEUS)in the differential diagnosis of intrahepatic cholangiocarcinoma (ICC) andSLI-RADS category M (LR-M) hepatocellular carcinoma (HCC), and to evaluate the diagnostic efficacy of the model. Methods: The contrast-enhanced ultrasound (CEUS) data of pathologically confirmed ICC and HCC patients were analyzed retrospectively. All the nodules were classified by CEUS LI-RADS. The propensity scores were matched according to the nodule size (ICC:LR-M class HCC=1:1). The patients were divided into the training set and the validation set according to 7:3. Using R software, the most characteristic CEUS signs of ICC were screened by Logistic analysis,and the nomogram model was constructed. The ROC curve, decision making and the calibration curve of the model was evaluated. Results: Compared with LR-M HCC, the most characteristic CEUS signs of ICC were circumferential enhancement in arterial phase, intra-tumoral vein, blurred boundary of unenhanced area, and unclear enhancement boundary of tumor. The nomogram model with CEUS features is constructed. The AUC of the ROC curve was 0.9062(training set) and 0.8908(validation set), and the model had good discrimination. The calibration curve showed that the prediction probability of the model was consistent with the actual pathological results. The clinical decision curve showed that the model was clinically significant. Conclusion: The nomogram based on the characteristics of CEUS was of high value in the differential diagnosis between ICC and LR-M HCC.
