    摘要:

    目的:探索双切面(长轴最大切面、短轴垂直切面)超声影像组学在乳腺结节良恶性诊断中的价值。 方法:前瞻性收集2022年12月至2023年12月浙江大学医学院附属邵逸夫医院、云南省曲靖市第一人民医院两家三甲医院乳腺超声数据237例。将患者按照7:3比例随机划分为训练集166例、测试集71例,分别在长轴最大切面,短轴垂直切面勾画感兴趣区(ROI, region of interest),同时自动适形外扩3 mm,提取双切面影像组学特征,分别建立临床超声特征模型(模型1)、长轴最大切面模型(模型2)、短轴垂直切面模型(模型3)、长轴最大切面+短轴垂直切面模型(模型4)、长轴最大切面+短轴垂直切面+临床超声特征(模型5),通过受试者工作特征(Receiver Operating Characteristic, ROC)、曲线下面积(Area Under the Curve, AUC)、决策曲线分析(Decision Curve Analysis, DCA)来评价模型诊断效能。 结果:模型1在训练集AUC为0.89,测试集AUC、灵敏度、特异度、准确度分别为0.89、0.88、0.82、0.85;模型2在训练集AUC为0.87,测试集AUC、灵敏度、特异度、准确度分别为0.85、0.78、0.77、0.77;模型3在训练集AUC为0.90,测试集AUC、灵敏度、特异度、准确度分别为0.90、0.78、0.90、0.85;模型4在训练集AUC为0.92,测试集AUC、灵敏度、特异度、准确度分别为0.94、0.72、0.95、0.85;模型5在训练集AUC为0.98,测试集AUC、灵敏度、特异度、准确度分别为0.94、0.88、0.95、0.92。模型5诊断效能最高,Delong检验P<0.05,差异有统计学意义。 结论:基于长轴最大切面+短轴垂直切面+临床超声特征(模型5)诊断效能最佳,可以更好地辅助超声医师进行决策,既可以有效地降低延误治疗人数,又可以减少不必要的过度治疗,受益于患者。

    Abstract:

    Objective To explore the diagnostic value of double section ultrasound radiomics (maximum section of long axis and vertical section of short axis) in breast cancer. Methods Breast ultrasound data of 237 patients from two top-three hospitals, Affiliated Sir Run Run Shaw Hospital of Zhejiang university School of Medicine and Qujing No.1 Hospital, Affiliated Hospital of Kunming Medical University, were retrospectively collected from December 2022 to December 2023. Patients were randomly divided into a training set of 166 cases and a test set of 71 cases according to a ratio of 7:3. ROI (region of interest) was delineated on the maximum section of the long axis and the vertical section of the short axis, respectively. At the same time, automatic conformal expansion of 3 mm was performed to extract the image omics features of the double section. The clinical ultrasound feature model (model 1), the maximum long-axis section model (model 2), the short-axis vertical section model (model 3), the maximum long-axis section model + the short-axis vertical section model (model 4), the maximum long-axis section + the short-axis vertical section model + the clinical ultrasound feature model (model 5) were established respectively. Through the Receiver Operating Characteristics (ROC), Area Under Curve (AUC), Decision Curve Analysis (DCA) to evaluate the diagnostic efficiency of the model. Results For model 1, the AUC of training set was 0.89, and the AUC, sensitivity, specificity and accuracy of test set were 0.89, 0.88, 0.82and 0.85, respectively. For model 2, the training set AUC is 0.87, and the test set AUC, sensitivity, specificity and accuracy are 0.85, 0.78, 0.77 and 0.77, respectively. For model 3, the training set AUC is 0.90, and the test set AUC, sensitivity, specificity and accuracy are 0.90, 0.78, 0.90 and 0.85, respectively. For model 4, the training set AUC is 0.92, and the test set AUC, sensitivity, specificity and accuracy are 0.94, 0.72, 0.95 and 0.85, respectively. For model 5, the training set AUC is 0.98, and the test set AUC, sensitivity, specificity and accuracy are 0.94, 0.88, 0.95 and 0.92, respectively. Model 5 had the highest diagnostic efficiency, and the Delong test showed P < 0.05, the difference was statistically significant. Conclusion Based on the maximum long-axis section + short-axis vertical section + clinical ultrasound features (Model 5), the diagnosis efficiency is the best, which can better assist the sonographer to make decisions, effectively reduce the number of delayed treatment, reduce unnecessary overtreatment, and benefit patients.
