    目的 本研究旨在应用超声技术探讨心室-动脉僵硬度对射血分数保留的糖尿病患者心血管功能变化的作用及其变化规律。方法 选取79例射血分数正常的糖尿病患者为病例组(DM),58例健康人为对照组。测定有效动脉弹性(arterial elastance,Ea),左室收缩末弹性(end-systolic elastance,Ees),以及心室-动脉耦联指数(ventricular-arterial coupling,VAC),比较两组差异。结果 DM组Ea较对照组明显增高(2.03±0.41 vs 1.86±0.62 mmHg∕mL,P<0.05),差异有统计学意义。DM组Ees和对照组比较(2.45±0.87 vs 2.40±1.04 mmHg∕mL,P>0.05),没有显著性差异。DM组VAC较对照组明显增高(0.90±0.26 vs 0.81±0.17,P<0.05),差异有统计学意义。结论 糖尿病组VAC与对照组存在显著差异,超声能够直观定量、无创分析糖尿病患者心室-动脉系统功能匹配状态。

    Abstract:

    Objective To assess the role of increased ventricular-arterial stiffening on the left ventricular function in diabetes patients with preserved ejection fraction by ultrasound. Methods Seventy-nine diabetes patients and fifty-eight healthy subjects were enrolled as healthy control group. Arterial elastance(Ea),end-systolic elastance(Ees) and VAC were compared. Results The Ea of DM was higher than that of the controls(2.03±0.41 vs 1.86±0.62mmHg∕mL,P<0.05),which suggested arteries stiffness increased significantly. No significance of Ees was found between two groups(2.45±0.87 vs 2.40±1.04mmHg∕mL,P>0.05). VAC significantly increased than that of the controls(0.89±0.26 vs 0.81±0.17,P<0.05).Conclusions? Ultrasound provided a quantitatively and noninvasive method analyzing the state of ventricle-arterial system in diabetes patients.VAC in diabetes patients significantly differed from the controls.
